Did you put DEX-FLASH.EID0.NORBIN/DEX-FLASH.EID0.NANDBIN on the root of your FAT32 formatted flash drive and plugged it into the right USB port?

Make sure your extension is ending in either .NORBIN or .NANDBIN, I know I had this problem due to the file still being under .BIN :/
