I have 4.50ITA Dex (With 4.60 Spoof)

It's possibly the tool that you are using. Maybe the debug eboot you think you have on it not really debug.
If the game is MW3, it doesn't work for me either, use ccapi, I also do that.
Check the debug settings for:
boot mode: system software mode
release mode: development mode

Its a tool i made. It worked yesterday, but idk why, today doesnt attach.
TMAPI is connected. But idk why i cant attach now.

If you did use this reply back I maybe able to help you fix this problem also dont setup ccappi atm.

tapi works fine for me with that installed. Maybe it's because I have ccapi installed lol
