I dont know what specific CFW you have, but It happened to me on 4.50ita. The problem was the spoofer I was using was interfering with target manager causing ps3 to crash. The fix is to delete "4.66 Spoof Enabler-Disabler v1.11 by arch" and instead install "4.66 Spoofer for PS3ita" found her on NGU.
