Try and use a different usb as there may be a problem with it. Also try plugging the usb into a different port. And if it does read it, make sure the files are in label folders so it can be read by the system

Is it placed in the following order? usb/PS3/UPDATE/PS3UPDAT.PUP Make sure everything is exact, even the CAPS>

ur files may not be named correctly & what cfw are u on?

Try renaming your files and folders correctly.
Also if you're doing it through XMB give the Safe Mode a try as well.
