Normally only the CID, but first get a antiban eboot. That's why you are continuing to get banned.

Ok I'll try that

Make sure it has antiban, then you need to change your CID again. It doesn't unban you just prevent it. Preferably a private and not public CID as they tend to get banned fast.

If I get banned again, I need to change the mac, right ?

I'm on an infinite connecting to online services screen when I go online after loading the game
If I go online from the XMB I can go online just fine
Any fix ?

Just launch the game signed in, but make sure you have antiban first. A MAC address can be pretty much made up by anyone, normally just change the last 2 characters. But no for bo2 you shouldn't have change it. Only the CID. I'm pretty positive on that, so just change your CID and use an antiban EBOOT, not from a tool and launch the game signed in. Should work fine after that.

With that you shouldn't ever be banned :p just remember if a new update is released you will need to get an updated eboot.
