Load it through multiman, it should prompt you saying would you like to apply patch or something, hit yes.

I would suggest you torrenting it. (don't ask for a link warez isn't allowed on NGU)

Just download the BLES02166 version of black ops 3. FTP the whole folder into dev_hdd0/GAMES and launch it with multiman. It will ask you to patch the game if you're on 4.75 or lower. Also make sure to mount the game and select "check for updates" that will most likely fix it. OR you can try resigning the eboot to 4.21+ NPDRM EBOOT.
