Well is your update named PS3UPDAT.PUP and it should be inside a folder named UPDATE that's inside a folder named PS3

Just a swing in the dark here, did you try installing rebug 4.78 D Rex from 3.55 xmb?

do you have the usb in right usb slot

1. Make sure you have in right slot
2. No discs
3. Inserted properly
4. Re-download the file and put it on usb. Safely eject!
