Create the following folders/directories in your USB:
PS3 > UPDATE

Then put the file named "PS3UPDAT.PUP" inside the "UPDATE" folder. It should look like:
PS3 > UPDATE > PS3UPDAT.PUP

Then plug your USB in the right-hand port of your PS3, go to Settings > Update Utility, and select "Update via Storage Media". Then select the "PS3UPDAT.PUP" file.
