ppl always have all kind of theories, but this is mine just dont go online and online and troll ppl to hard to the point where they want to report you, other than that you should be good the only games ive heard u might need to disable the Sycall from is destiny and BO3, but im speaking from experience ive only been banned twice within 3yrs, and i didn recoveries and all, only the 2 times ive gotten banned was a couple after i did my trophies.

You may have an outdated app, whatever you're using to enable ofw mode. But it's not needed, it's optional. I've never done it and have lasted 2 years, just play smart.
