It's an eboot error. Are you using modded files? What CFW are you on?

Yes I am but I changed the GTA 5 and BO2 eboot back to original and don't work.

Do you have debug eboots on your games because you're on dex? And do you mean download from PlayStation store or from the internet and loaded from a backup manager? If it's in a backup manager, make sure you have the settings internal/external, depending on where your game files are, and bd mirror. If it is from the PS store, just make sure you have the debug eboots which you also need if you are loading it from a backup manager.
