Hmm . the only time i have had this is when my system froze, it deleted all saved data , and left the * buy username , if this is what happened goto debug settings and change np enviroment to - np
this caused me alot of issues , as for the safe recovery option that will cause this - i dont think there is one , i have used all the options and never managed to make it happen again , so could well be the hdd giving a fault .
