You have to create a folder on your USB. Do these steps...
Step 1: plug in your USB device to your computer.
Step 2: Delete ANYTHING you may have on it.
Step 3: Click New Folder, and call it PS3
Step 4: Double click that folder, then click on New Folder again, and call it UPDATE
Step 5: Double click that folder, and then click on New Folder once again, and call this 3.55 CFW
Step 6: Click that little arrow that's pointing up to the bottom right of your Windows 7 screen, and then click your USB device, and select 'Safely Remove'....

i did that there is nothing in the install pkg folder i even highlighted the install package files, it scanned and said no installable package was found. i dont think i can install with ps3ita.

To put his PUP file on your USB, simply place it on the root of your flash. Make sure you go ahead and DELETE EVERYTHING OFF YOUR FLASH DRIVE, before you do. Then just plug your USB into your console.

{Make sure you have ALREADY INSTALLED 3.55 OFW BEFORE YOU DO THIS}

Now to put OFW 3.55 on it, you have to go into recovery mode to install the original 3.55 firmware, if you haven't already.

Then once that's done, plug in your USB flash with Geohots CFW, and go over to Settings>System Update>Update via Storage Media.

OMG i was on ofw 3.55 and then i put PS3ita 3.55 cfw on, so inorder to get PS3ita off i have to go to recovery mode and reinstall OFW 3.55 then put geohot's on.. ok how do u put a slim into recovery mode.

lol...To put the slim into recovery mode, do this.

With the system turned OFF, place your finger over the power button, and HOLD IT DOWN until you hear it beep once...KEEP HOLDING. You'll then hear it beep one more time again, and it will shut off.
Remove your finger from the power button, and then HOLD YOUR FINGER BACK DOWN ON THE POWER BUTTON.
You'll hear it beep once...then it will beep TWO TIMES.
Remove your finger, and you'll be placed into recovery mode. =)
