Complete chapter 1 on the Hardest Difficulty in the Single Player Campaign
Freedom - Guerrilla
Complete chapter 2 on the Hardest Difficulty in the Single Player Campaign
Fire Sale - Guerrilla
Complete chapter 3 on the Hardest Difficulty in the Single Player Campaign
The Wall - Guerrilla
Complete chapter 4 on the Hardest Difficulty in the Single Player Campaign
Heartland - Guerrilla
Complete chapter 5 on the Hardest Difficulty in the Single Player Campaign
Overwatch - Guerrilla
Complete chapter 6 on the Hardest Difficulty in the Single Player Campaign
Golden Gate - Guerrilla
Complete chapter 7 on the Hardest Difficulty in the Single Player Campaign

UPDATE - 09 June 2019:
Removed Untested prefix from thread.
Got the time to create a new account and test the saves,
all the above said trophies unlocked for me.
I used Bruteforce to resign the saves.
Found out another trophy is earned....
Safer Skies (Should pop a few moments after the game starts)
Destroy all the SAM trucks in the level in Chapter 6: Overwatch
I've also added that to the list above.
Removed Untested prefix from thread.
Got the time to create a new account and test the saves,
all the above said trophies unlocked for me.
I used Bruteforce to resign the saves.
Found out another trophy is earned....
Safer Skies (Should pop a few moments after the game starts)
Destroy all the SAM trucks in the level in Chapter 6: Overwatch
I've also added that to the list above.

Any chance this go to PS4 ? I like the game but the hardest difficulty trophies will be a great help to me

These game saves are for Homefront (which ONLY came out for Microsoft Windows, PlayStation 3 and Xbox 360)
You're getting this mixed up with the Homefront: The Revolution (which came out for Microsoft Windows, PlayStation 4 and Xbox One)
