Where is this save located at in the game:

Ex Hard Mode meaning I defeated Chaos once already in the boundary plane and cleared out some of the extra rooms that spawn in ex hard mode.

Yeah cause I finished EX Boundary Plane for conqueror of the planes trophy but I didn't get a title for it for some odd reason I'll upload that save here for those who are missing it shortly.

Also does the gallery trophy unlock along with the titles trophy?

didnt find any offset count for titles earned.not like agarest zero.so i think have to mod all 146 titles manually to make it work.

I found my old save on my ps3. If you want to mod my character like you did on the other save then its better because I have all titles done except 3 or 4. It will make the titles trophy alot easier for others.

Which weapons are you missing for the titles? Do you remember?

Just look in the weapons list. I am pretty sure I am only missing a few weapons which can be either stolen or gotten from overkilling bosses in the boundary plane.
