what trophy hack are you doing ? if you are trophy hacking using REBUG , THEY WILL NOT SYNC TO SERVER

start the game and it will sort itself out on the next sync, thats only if your on a new ps3 and its the first time youve put the disc in. sometimes that happens but its fine when its sync with the server
