Before starting, make sure you are in OFFLINE MODE.

Now, with that said let's go kill this beast.
The save places you right inside the Lament Chamber with the Ur-Dragon having only 1/2 a bar of health remaining.

Head to the back of the chamber and kill the 3 pawns that are there.
Once done, wait a few moments and the Dragon will spawn in.

Now, this is very important: Hit select to open your inventory. Scroll down to tools and equip the Blast Arrows. Each person has a shit-ton so you won't run out.

Now, the last hearts remaining are on the top portion of each wing. I suggesting using the square ability Fivefold Flurry as it will do a noticeable amount of damage.

One other thing to take note of is you cannot die!
I used all my Wakestones fighting him. One of my pawns has 50+ curitives if you need healing.

Once killed the trophy will pop!
