play last level from chapter 1 to get

play last level from chapter 2 to get
san francisco is safe, next!
team victory
play last level from chapter 3 to get
could we fight something bigger?

play last level from chapter 4 to get
what alien menace?
team supremacy
not a novice anymore
Remember the Heroes
Stay put and watch (don't skip the final cutscenes and credits)
complete any monster challenge, earn any medal for:
olympic champion
gold olympic champion
Use save slot 2(mysave) to get:
buy remaining nodes on dna lab to get
art aficionado
shopping frenzy (3 nodes left)
full arsenal
two is better than one
misc trophies (use save slot 2):
flying fish & could i be any better than this (finish off 300 enemies with missing link, use monster challenge 4, and do leap attacks)
that's a blast (connect two controles than hold x on second controller to use charge blast)
strong attraction (when playing with 2 controllers, press triangle on 2nd controller and use it on an enemy)
Collateral damage (destroy 30 helicopters on ginormica levels with 2nd controller)
air support master (destroy 100 bholders with 2nd controller in missing link's and bob levels
maximum atraction ( the same has strong atraction, do it 50 times)
get out of the way (monster challenge 6, make sure you use chain dash)
strike with attitude (play monster challenge 4, hold square for 3 sec then attack)
expert pitcher (throw 25 enemies with missing link, not tested)
around the world, pinball fan (redo monster challenge 13 complete it)
machine gun specialist (redo chapter 1 scene 3)
safety first (play monster challenge 5, press l2 when it starts)
casual collector & never say enough (start a new game or use your own, play level 12. in the belly, more info here: You must login or register to view this content.)

I tested it just now,it worked, "Remember The Heroes" trophy does pop,try not to get died in scene 25 anyway.

Did you play the last level of the 3 previous chapters before in order to pop the other don't die more than 10 times in the X Chapter ?
