Just play the last Mission "End of the Line" to get the Trophy.
The End of the Line
Complete End of the Line.
---------------------------------------------------------------------
When you finish the Mission dont skip the Cutscene and the Credtis
to get "Time to Kill" / Watch Credits to the end.
---------------------------------------------------------------------
Maybe other Trophys can pop too:
"Original Gangster"
Reach maximum respect.
"Grove Street War Lord"
Capture 30 territories.

just tested it those two didn't unlock only the End of the line and Time to Kill trophy, for what I understand you already unlock Original Gangster and Grove St. War Lord during your gameplay that's why it didn't unlock.
