A lot of people are posting the same theories and mixing up the emblem with the COD4 one. The COD4 one, which is shown below is obtained through the Spectre IV challenge completion:

THIS IS NOT THE EMBLEM IN QUESTION. The emblem we're trying to unlock is this one:

According to 402, this emblem is obtainable in the game without hacking or modding. Proof of him saying this is shown on his Twitter account as well as the pictures below:

So now that you all know what emblem we're looking for, here are some of the THEORIES that people have thought of and proven to be wrong. Also, no one has obtained this emblem without hacking the game.
THEORIES IN THE TRASH:

The game doesn't know the difference between hack and legit.
All Achievements Unlocked: FALSE.
100% Completion on Campaign, Spec Ops , Multiplayer (1100%): FALSE.

This would take approximately 112 nukes but if you want to be realistic and say 6 kills per game, you would call in about 167 nukes)

Complete all HIDDEN Challenges: Hackers have pretty much found all of the hidden challenges in the game and completed them. The game doesn't consist of many hidden challenges and even if it was a hidden challenge, then modded game lobbies would unlock every challenge despite it being hidden.
So there's the few theories many of us have gone through in order to get this emblem, and thus far, we're pretty much out of luck and ideas. Hackers have looked into the .csv (game codes) to find something that may stand out, and so far the only thing they found is the following:
"level 70 and pr_marksman_ak47_4"
You have to be level 70 to unlock the AK47, unfortunately the Marksmen challenge only goes upto 3 (III), which doesn't help us at all.
It also doesn't have anything to do with any of the DLC content as 402 said that the emblem was obtainable on the PS3 way before the first DLC came out.
Post any new theories you guys have. We're going to find out how to get this emblem!
Also, please refrain from posting theories that have already been proven wrong. We need NEW ones.

u could only hack on 360. might be possible on ps3.

before the 1.10 patch we could create a private game with the team entering HQ veteran ranger skidrow and we like the opening phrase "thermo nuclear war" even if the game was held in HQ! but now that they have enabled the veteran HQ .... if we create private game the same way as before .... does not say more thermo nuclear war! I believe that 402 there is a little kidding! and I think the spinning skull to unlock that way! playing the thermo nuclear war! or at least is my idea! " I wonder why this mode is locked! there will be other surprises? Then I asked why special operations mode there is also the playing time of IW! I have 3 stars in the mission where there are the shapes, the first mission in alpha! and beyond to give me my time to complete the mission ... I also from time fully mission of IW! this makes me think that it is a challenge! Moreover, how to be more skilful than them if we can not play online against them all? there is to beat their time? if you hacking trophies ... You have all the stars in every mission and theres their time! more in one of the last missions .... says "IW time. only used the knife and grenades," it seems to me very like a challenge! I do not know!:confused:

it would help if the sentences made sense? sorry your ideas are scattered, and i couldn't understand a whole lot. are you saying that maybe we have to beat IW's time in all spec. ops and single player maybe? and maybe it has to do with global thermonuclear war? and you think he's lying to us about the emblem, that it's not unlock-able yet?

one thing i could think of that hasnt been posted yet or isnt being tested by you awesome guys out there is that it could be like getting a headshot with everyweapon in spec ops. Or possibly not missing shot in spec ops <------------ pretty ****ing insane, i know. Or maybe you have to have your accuracy and win percentage above a certain number in multiplayer....??
