It 's pretty obvious man.. Simply means your computer isnt good enough to handle the settings your playing on. You'll need to crank down the quality you play New Vegas with.

Make sure you have Vsync off, majority of people who game prefer to have vsync off because of performance issues such as the one you are speaking of.

There is an easy way to fix this, all you have to do is to add

fForegroundMouseAccelBase=0
fForegroundMouseAccelTop=0
fForegroundMouseBase=0
fForegroundMouseMult=0

at a certain point in two .ini-files. The first file is located in the F:NV-folder and it's called Fallout_default.ini. You have to search for the [Controls] part in the .ini and then just copy-paste the above lines at the end of that block.
The second .ini you have to change is located in your My Games directory. Somewhere like C:\Users\<your name\Documents\My Games\FalloutNV. This one is just called Fallout.ini. Make sure that the file isn't read-only, search
again for the [Controls] part in the file and add the lines.
Congratulations, you are now mouse-lag-free.

This worked great for me, and i am running it on a crappy toshiba laptop.

just run it on medium settings you should never have trouble i run at max with a 4 year old computer
