I think it's - Dark Souls, Demons Souls, GT5

The worst one I am having the most issues with is White Knight Chronicles 2 and that is the Soul of Evil trophy. It is the only one I need for a plat... so sad. been working on it for about a year and a half.

From my experiences Metal Gear Solid: Guns of the Patriots, but I'm sure there's much harder Plats out there.

Dark Souls, MK vs DC universe, TLOU ^_^

Killzone 2 was one of the hardest plat I ever made. And totally legit.
The second is Wipeout HD, but I admit I used the save here to get the Elite trophies. But beating Zico was fuc*** hard xD.
I plan to plat Demon's Soul but I tried the game and died after 2minutes and cannot understand how the game works 'cause I'm stuck...
