Obviously there are some awfully made games that remain generally unknown to most gamers, so I doubt anyone could accurately label a single game as the "worst" (as they haven't played through every game available on the console). However, I can select a "worst of the best and known": Final Fantasy 8, Chocobo Racing, Thousand Eyes, and Dragon Quest 7 would probably be my candidates.

Bubsy 3D is still to this day one of the worst games I've ever played.
