You need to play every mission it doesn't matter on what rank you will finish it will automatic turn into a S rank
that is a good idea actually, can you make a code where hell and hell difficulty trophy pops no matter what stage you play? or play mission 20 on any difficulty and difficulty trophies unlock, i'm sure that could be done instead of the gold orbs.
