I haven't played any Diablo past the second one, years ago, so can only give some very general advice. By the way, while I can't tell for sure it looks like you're going for the "Reign of Terror" trophy, so I'm assuming that for all of the below:
Is there an option to Load Game as opposed to Continue Game? It looks like the save here is an autosave but sometimes it still helps if you manually select the autosave.
I'm not sure what you mean by challenges, since this looks like a difficulty-based trophy, but do you have to have the challenge selected?
Have you checked the save, before you go on to beat the game, to make sure it's on Hardcore difficulty?
Last thing I can say, and sorry if this seems obvious, but I'm just trying to throw everything out there, but there are 2 saves here. You say you can only get the Malthiel trophy, but that's the first save. The second save is for Reign of Terror. Maybe double-check that you downloaded the correct one? Again, I haven't played this one so I'm not even sure where the saves are supposed to put you, in-game, but maybe you could tell just from where you are when you load the save.
Also on it being 2 saves: If one doesn't work, try the other one. Both are on Hardcore difficulty. If one cannot work because the challenge is already complete, maybe it's not yet complete in the other save? It may take some work to get from Malthiel to Diablo, but it's better than nothing. I'd still check first that you downloaded the correct save - the one marked Reign of Terror - first though.
