The safe doesn't work for me? Everytime i click on continue the safegame is not on hardcore... it's on normal difficulty. Finished it two times now. No trophy...

I have the same problem
Meaning you hit "continue" and the game's not on the correct difficulty? A couple pieces of general advice:
If there's a way to manually pick which save you're loading, rather than "continue" which usually means the game is picking itself which save it's loading, then use the manual load instead.
Also, make sure you're starting up offline. Game servers sometime store some information server-side and that can mess up offline saves if there's a difference.
If nothing else works, then back up all your Metro Exodus saves somewhere. Then delete them off your PS4, even your profile save (if there is one in this game). Then resign this save again, copy it over so that it's the only save for this game on your PS4. Start the game offline, then load your game. Sometimes just the profile save alone can mess things up, and other times it's quite necessary. So try it both ways.
For a few games, I've noticed that difficulty saves won't be set right unless you've already either started the game on that specific difficulty, or already beaten the game on whichever difficulty unlocks the harder difficulty to begin with. To test the former, just start up a new game in Ranger Hardcore, get to a point where you can save, save, then try this save but make sure you keep your profile save you had. To test the latter, try beating the game once using the difficulty the save loads in, then load it up and beat it again.
