I made some saves for [EU] Tom Clancy's Ghost Recon Wildlands [CUSA02819]. Tested on my dummy account.
These saves should work on US region. Some users reported that they're working. I didn't test it on US region, because I'm EU.
Here are the saves.

Save #1.
Serious Collector.
Start the game OFFLINE!
Collect one more document.

Save #2.
1. Heavy Medals.
2. The Whole Story.
3. Legendary Hunter.
4. Submachine-Gun Fanatic.
5. Assault Rifle Fanatic.
6. Light Machine-Gun Fanatic.
7. Handgun Fanatic.
8. Sniper Rifle Fanatic.
9. Shotguns Fanatic.

Start the game OFFLINE!
Collect last document, legend and bonus medal in Montuyoc region.

Last machine gun model Mk249 and pistol model P227 can be found in Malca region.
Last shotgun model SPAS-12 can be found in La Cruz region.
Last submachine gun model 9x19VSN can be found in Inca Camina region.
Last sniper rifle model L115A3 can be found in Monte Puncu region.
Last assault rifle model TAR-21 can be found in Montuyoc region.

UPD #1. 24.05.2019
The Whole Story trophy won't pop up on 1.28 version of game. Unfortunately, there is still no solution for this.


UPD #2. 25.05.2019
Solution found. With 1.28 patch Ubisoft added new mission Operation Oracle. You have to find 5 more documents in this mission and 2 documents in Montuyoc region. After that trophy should pop up. Na not yet. But someone said they tried this method (Back in 2017/201Cool Man (aka Tustin) and they got it to work. Try this and let me know of the results because I can't get it to pop.

Download a platinum save from here and resign it. Turn your internet connection off and copy it into your ps4 and then start game and load up the save. Once you are in game, exit out to main menu and start a new game in slot 2. Just go and collect another document and apparently it's meant to pop

Ok, guys, I tested save again and it don't work. And I know what the problem is. Because Ubisoft released crappy 1.28 patch on the 30th April that brings some new bugs. I tested my saves on previous version of game on the 28th April and it works fine. We have to wait new patch, there is still no solution for the Whole Story.

A few options come to mind, but first, thanks for getting to the root of the problem. I miss the days when patches came out along with expansion packs - for those who remember what those were (think DLC the size of games) - and a really buggy game might go up to version 1.04.

All of these options assume that the saves you've uploaded here are from 1.27 or below, or whatever patch number still works. Or if these saves are already updated to 1.28, perhaps you still have the pre-1.28 saves lying around.

Also, if an online connection is required for whichever trophies are problematic (I'm not familiar with the game), this wouldn't work either.

-1- If someone has disc version, probably simple enough unless a Day 1 patch (or a later patch) is necessary. Just uninstall and reinstall from disc.

-2- If digital version, use a PC as a proxy server, run a program called PSX Download Helper, google a guide to help you set it up properly, and you can often download version 1.00 of digital games from PSN. Again, if a Day 1 patch is necessary, or if need a later patch for a DLC trophy, this won't work.

-3- More complicated, but the only way I'm aware of to get particular versions of a PS4 game to download rather than either the latest version or 1.00, would be to use set PC as proxy, run Charles on the PC, and within Charles replace the address or addresses of the patch you don't want with the particular patch (such as 1.27) you do want. This can even work for 1.00 if PSX Download Helper runs into trouble, which is sometimes does. Problem is that the various databases of URL addresses for PS4 game patches is very far from complete.

Guides for setting up and using PCs as proxy servers for PS4s are all over. I've seen a couple of good guides for PSX Downlod Helper. Same for Charles, and I think one of them is one this site. Method 3 has been done most famously for Final Fantasy 15's Comrades DLC trophies, which are unobtainable without this method. Guides that show you how to get those trophies now, after patch 1.27 of THAT game, would probably also link to the lists of various URL addresses for PS4 game patches (they are different for each region).

Can you try again but use this save instead and follow something different.

Just resign the save and copy it. Start the game and press continue. Then exit back to home, select "select save campaign" and pick slot 2 to start a new game. Then collect one document and see if that pop as this save is from 2017 which is much older than Ops save

Ill also try this out. Will report back with my results. Thanks for the heads up

Edit: Just tested and can confirm it indeed works. The files are not at all difficult to find. My trophy triggered when I opened the door to find the punisher guy and the cutscene initiated. Objective was "Find the engineer"
