Do a barrel roll!
There's two saves, 100% on normal and the other one on hardcore.
Tombs completed 100% on normal, and two tombs on hardcore.
Replaying tombs via challenges should give gold trophies for completing it, and 2 trophies for time/score challenges. Wearing Lara required items should give trophies.
You can't get story (main or dlc) trophies, as there's no mission replay at all. DLC missions require almost 100% game completion.

I found it i was doing now the 2 tomb challenges in san juan again in challenges mode but no difficult trophys pop up
I only left 3 Trophs for my Platinum .... Rite of Passage .... Deadly Obsession and ... One with the Jungle
