Warning: None of the saves are tested, all missions are completed on Veteran in the 45 Intel Items save so you may pop the trophy for completing the game on Veteran if you finish the mission.
Instructions for the Intel Items: just load up the first mission [S.S.D.D] and get the Intel Item in it
you can also get veteran trophies and "Immortal" trophy using the platinum save in this set.

Completing every mission on recruit difficulty will also give you the veteran difficulty trophy for that particular mission or set of missions .. for example if you want the Prisoner 627 Complete 'The Only Easy Day... Was Yesterday' and 'The Gulag' on Veteran difficulty trophy , then simply complete the gulag mission on recruit difficulty to get that trophy .. its recommended to complete the game once on easy difficulty so nothing pops out of order

Does that mean restarting the last checkpoint of the save or redoing the entire last mission?

To be more accurate, what I did was go to mission select and restart Endgame on Recruit. After Price shoots down the chopper and you both fall down the waterfall, Immortal popped up for me
