basiclly, load the save and R2 to finish the final part of the story
save time/trophy/instruction.
23:11 Teller of Tales finish the story
0:26 The Headstrong Thief finish the story(yona)
0:40 The Ghost of Legend finish the battle
2:49 The Vengeful Warrior finish the story(masako)
19:53 The Unbending Archer finish the story(ishikawa)
this save should have 19 mongol artifacts, so you can find 1 more to get 20 artifacts trophy
3:45 Favor of the Kami load the save,R2 to honor the last shrine
The Warrior Monk/Helping Sword Hand fast travel to the last mission to finish the story.
Light the Way I leave the last lighthouse(#7 in powerpyx guide)
Cooper Clan Cosplayer change sword to sly takumi
this save should have 19 records, so you can fins 1 more to get 20 records.
Honor the Unseen Bow to 10 hidden altars across Tsushima(sorry, have not do it yet)
Body, Mind, and Spirit/Complete all Hot Springs, Haiku, Inari Shrines, and Bamboo Strikes. forget to leave, but you can use save 19:53, only few left to get trophy.

yes, you should delete sec_sdmemory files, i think it is auto-save.
and you should not choose continue the game, choose load save. for sword trophy, just select the below one, others already equipped.
i tryed so many time with diffrent ways and still the trophy did't pop

sorry for it, but im really don't know why it happen...as it is the last trophy to plat in this plat through.
as other thread may have this trophy save, you can try for it, sorry for the inconvenient
