Use WinRAR to extract it. I've heard a lot of people following all steps precisely wind up having trouble after using different programs. WinRAR is a free program. It will keep telling you your trial period has expired. Just close the prompt. You can keep using it.
After extracting the RAR, with this particular download, you will have a CUSA folder and there will be PS4 files inside of it. So it will look like this:
CUSA folder >>> PS4 save files
You have 2 options. You can either create a ZIP file, or you can put that folder directly on a USB stick. I'll describe both briefly.
1. To create a ZIP file, assuming you have Windows 7 or later, right-click that CUSA folder and a context menu will pop up. Hover over "Send to..." then "Compressed (Zipped) Folder". The exact language might be a bit different between Windows 7, 8, 8.1 and 10 but not by much I don't think. WinRAR can also create ZIPs but this way is a bit faster. So now you have this:
ZIP file >>> CUSA folder >>> PS4 save files
Go into Save Wizard, go into the Resign menu, click "import" and import that ZIP file. That import button can ONLY read Zip files, nothing else. Not RARs, not CUSA folders, only ZIPs and only ZIPs that are in the exact format I just described, with the CUSA folder right inside and PS4 save files right inside of the CUSA.
Remember to resign EACH file in the ZIP. There might only be one, but if there is more than one, do all of them. Then transfer them to your PS4, and keep the PS4 offline when you load the game and load the saves.
2. You can also skip the ZIP and resign it directly from a USB stick. To better describe this, if you copy saves from your PS4 to a USB stick and look at them on a computer, you'll notice that they are saved like this, where E can be any letter, maybe D maybe F it depends on how your PC assigns drive letters but it stands for your USB stick:

The "1234..." is actually any random string of 16 different numbers AND letters, and is your profile for your account. I just picked a random one. If you set up Save Wizard properly, then Save Wizard already knows that string of 16.
You're going to make up a fake one. An easy one is a bunch of 0s. I use 16 zeros, but I've seen some people use just 1. I'll use just 1 to make it easier here.
Navigate to that "Savedata" folder and create a new folder. Name it 0, or whatever else as long as its new, fake and you'll be able to tell it apart from your real one. If it's only 1 digit away or otherwise looks real, it would be tough. Leave your real folder alone.
Now put the CUSA folder that was inside of the RAR, from the download, inside of that new, fake profile folder. The downloaded save should now look like this:
E: >>> PS4 >>> Savedata >>> 0 >>> CUSA folder >>> PS4 save files
Open up Save Wizard and go into the resign menu. You'll see the saves from the download directly in the menu. They will appear under profile "0". Do not assign a Save Wizard profile to your fake profile, since you only get 3 Save Wizard slots and you don't need to use one up for this.
You can resign directly from that menu. Don't import. Just click on them, maybe right-click (can't remember). You can delete them as you resign them which makes it a bit easier to keep track of multiple saves for larger download sets. You'll start seeing the "0" in the profile get replaced by your actual profile, however you decided to have it designated with Save Wizard.
