Answering the questions.
1) You will be able to choose either to use your real name (maybe the one you use on Facebook and/or the one you set up with your PSN) or your Username as a Display Name.
2) PS3 Accounts will be carried over, and yes, PS+ also. The only rumour we heard is about the Gold, Silver, Platinum and Diamond membership, who knows what that is about if they said PS+ will be carried over.
