how do you do it then

You can hack it using Xploder. However GTA4 is only semi-hackable. Online will need to be done legit and now that Megaupload has been taken offline, no guarantees that anyone can find the saves that make the other stuff easy (Brucie's races, 100%, pigeons and so on). Plus there are story trophies so the main game will need to be played legit.

GTA4 is by no means a fully hacked game unless someone goes out of their way to make a save for every single offline trophy, which can't be done.
