This is the 1.3 patch for Call of Duty 4: Modern Warfare.
This patch addresses the issue reported by community member Uzi Kidding which now catches some files bypassing the pure client test. In addition this update will set properties so that Anti-lag is always enabled upon map loads, an optimization for Sniper Rifle accuracty at certain distances, as well a fix so text chat between rounds of multiplayer will no longer cut out at the end. The 1.3 patch will also include all previous fixes from previous patches.

I don't think we will be doing that way any longer, I have seen quite a few people claim to still be able to do it, but who knows.
I started with 64, and did some online hacking with ps2.
If it is no longer possible to modify the info on the HD with a binary editor, and just resign it, than I don't see how it's possible.
UNLESS, you put the game file on the 360 HD (so that you could boot game directly from HD) , Uploaded that original game file to PC and alter it, re hash and replace the original file with the modded one, boot it up on the 360 and there ya go. I am pretty certain this would work offline, but I think the people at M$ and IW wouldn't make it that easy to do online. Would they?
The big problem with most hackers is they go crazy with it, making it painfully obvious and abundantly available to the public getting the attention of M$ & game producers, then forcing a patch. I mean it is cool to share with a select few maybe on a forum, that way you can at least have a few weeks of fun with it before it spreads, but the dumb asses post it on youtube and then we get a patch 2 days later
I am sure someone will figure it out again, and be dumb enough to post a viral video on youtube, just gatta keep those eyes peeled.
I am thinking about getting one of the transfer cables and play with it a bit offline, plus may be useful in the future
