like 2 of 2 unlocked where is all the achievements ????

Load your Black ops if that don't fix, delete profile in Settings>Memory>Profile>(your profile Name)> Awesome face

elete Profile Only. If this doesn't delete your profile first time then do it again. Then recover your profile this will reset all your achievements and fix them to what they were before they messed up
