This is mac/psid or only psid ban. Try some cc-cid and generate random psid, then try going online. If it doesn't work, you need to change mac.

Black Ops II won't ban console ID. They ban MAC Adress and profile.

You're going to need to downgrade to firmware 3.55. Then you'll need Chaos Air Bringer.pkg to change your mac address.
There should be a tutorial around here or just google it.

If you have qa enabled you don't need the 999 downgrader. just use rebug 3.55.4 rex to downgrade to 3.55. Once you're on 3.55 open up your rebug toolbox and find Dispatch Manager Access and enable it. Exit toolbox and start up Chaos Air Bringer.
There are tutorials on how to use Chaos Air Bringer. One last thing you don't need to completely change your entire mac address just the last to digits will do.

Treyarch doesnt ban cids they only ban mac, psid and accounts.
