It works offline but when I connect to online servers it always freezes and no matter what I try it does not seem to work

No I've made a new acc but there is always a symbol in the bottom left corner that pops up it sorts looks like a notepad but when that symbol shows up the game freezes.

Yeah, that symbol is making you freeze. It is like a box from from Black Ops 2 like a name change box. Normally it is on the account and you can make an account and be all good.
