Tip #1. Make sure neither of your code files are over 1kb. If they're 2kb or anything higher, your codes will not work.

Tip #2. People talk about going to split screen to make sure their codes work, well here's a shorter way. When you go to multiplayer, before you press play online, press the PS button - go to account management - Press triangle - press X on "Auto Sign-In OFF". Then exit xmb and hit play online, your codes should work. Just always remember to turn auto-sign in back ON because you have to do this every time to ensure your codes to work.

Look at their size on your computer, not on your ps3, the GPAD0_CM and GPAD0_MP should both be 1kb or smaller.

Since your hacking the online you can delete the SP (single player) my file always loads with out the SP and the total file is like 80kb
