Hey NGU,
I haven't posted on here for quite some time, but i just thought that I'd share my own strategy for 'five'. I've used this strategy to get to round 30+ many times and it's pretty much bullet-proof.
Basically, open everything up as usual, get a gun from the box (preferably galil, HK. RPK, AUG, or any of the superguns). What you want to do is on the second level - the defcon room - you want to open up both barricades on either side of the room downstairs. Now depending on how many people you have playing with you, this can be really easy or just a little hard.
How you go with this strategy depends on how well you are able to stick together with everyone else in a group. You need to stay really close together and as a group, run around the bottom of the room in the loop you just opened. I know its very tempting, but try not to shoot at the zombies until you can see no more coming in from upstairs. When they have stopped coming, I usually turn around just afew juggy and blast them. Mind you though, you have to be weary of where you are headed when walking backwards and shooting
FOR THE LATER ROUNDS, it gets alot harder. Around round 30, you'll have to pack a punch about twice a round and you'll most likely have 30k points which have built up. The best idea is to leave a crawler at the end of a round, then prepare the pack a punch by hitting all the switches on the blacony, and only one of the switches on the bottom. This just means that if you run out of ammo you can just hit the button and jump right in the teleporter

Oh yeah, and when you still have the crawler about, just spam the box...trade away your empty pack a punched raygun. Chances are that you will get it back from the box anyway.
Mid round pack a punching is really hardcore. If you are desparate, you need to have monkeys or it will take forever with the shotgun by the elevator. Basically you'll need to throw about 2 monkeys to give yourself enough time to hit the switches upstairs, then after that you can get the ones downstairs easy.
JUST REMEMBER that the upstairs balcony is designed to trap you. Other than that, this strategy is the best. As for the doctor guy, i wouldn't really worry. Just keep your crappy gun out when he grabs you so you can blast him in the back as he does his parkinsons shuffle through the labs etc.
