The patch is a different thing from the .pkg, the patch is inside the .pkg.

Basically all the MW2 patch does is replace the files on the hard drive with the new ones inside the .pkg, what they did was get the patch_mp.ff off the harddrive and replaced it again. Doing things with a signed .pkg from Sony is almost impossible, get a debug one though and you can do whatever you like

You can't sign PKGs , don't waste your time.
