Seeing as somehow loads of people have managed to get hold of this, despite the fact I only gave it to one person, I thought I'd just release it for everyone. So here you go, feel free to edit your own details in.

You need to download the .psd file in the first post of this thread then when you open it make all the layers visible, then select the following layers I'll say you invisible and then edit the text and adjust all the lyers by moving to this "side-tape" on the upper left side then replace the effects from layer 8 to layer 4.
