Well from what I've read from Google/PSX-SCENE you're screwed. One reply said try a different USB, but first format it to FAT32. Other than that I can't do anything.

Ok look in the USB and tell me your file/folder structure.
PS3\UPDATE\PS3UPDAT.PUP

MAKE sure you dont have "PS3UPDAT" as PS3UPDAT.PUP.PUP
Just have it as PS3UPDAT and make sure that file is a PUP.

Switch USB still don't work

did you run qa flag and have you verified that QA flag is enabled by going to network settings and pressing the same time:L1 + L2 + L3 (press left stick) + R1 + R2 + dpad_down??
Do you get edy viewer, debug settings and install pkgs????
did you do this: Debug settings>System Update Debug> switch to on

Is your blu-ray working????

are you doing this on recovery/safe mode????

Last thing you can do is enter FSM and install it.

Ok so here is the easiest way.use factory service mode with e3 card reader,if you don't have e3 card reader download ps3 factory service mode tool 0.2 for cex console.and than download lv2diag.self file 1 and 2 .download these files and follow the tutorial.and if you are thinking it will stuck in factory service mode.than your wrong.

1:Install the factory service mode tool 0.2
2:drag ps3updat.pup of rogero 3.55 v3.7 and lv2diag.self (file 1) in the root of a USB flash drive
3:plug USB in last port of USB near bd drive.
4:run the software in ps3
5:don't panic because of black screen it's normal.wait for near 5-10 min
6:your ps3 will shutdown and than take USB out and delete everything inside it.
7:drag lv2diag.self (file 2) in the USB flash drive.
8:plug in the ps3's last port near bd drive.and than turn it on.
9:wait for 2-3 min and than your ps3 will turn on.
NOTE:your ps3 will format using this method.
