All I have connected is the USB .

sorry meant to say a third-party controller...weird story is that I had a third party controller connected in the usb and the ps3 would not let me install and PKGs or UPDATEs onto the ps3. Once I removed the controller out of the equation everything started to work.

That is what I meant of course the usb flash drive is fine.

so its possible that your on something such as rogero downgrade pup, so all you gotta do is enable qa flag, install 3.55 ofw, then install a 3.55 cfw, then update to whatever 4.xx cfw you want
