If you have another playstation 3 i would try to put the hdd into it, and see if it would format it. Otherwise, your HDD might be ded.
Normally it would say "The firmware is incorrect, would you like to format..?"
If so, say yes then format it.. Then once its done, plug it back in your CFW PS3 and install CFW if it works.

Your hard drive its not dead is just corrupt so just try reformatting your HDD, remove from your ps3 and format ps3 HDD with your pc when done put back to ps3 and automatically your ps3 reformat again and install ps3 file back then install your same CFW again. And if this dosent work then you can try with different HDD,
