Theres a possibility that theres something wrong with your hard drive then. If you have an old or spare hard drive you could put in your PS3 to test then you should do that. It will save you the time and money of getting a new one if that isnt the case. Make sure you dont update though if you do it

Yes you need to install a cfw on it. If you were on DEX before then make sure you install a DEX firmware and same goes for CEX
