Are you using a clamp? I had the same problem and I was thinking I had the perfect connection but it wasn't.
I used a clamp and it got me perfect dumps the very first time.

No its not necessary. I got my dumps from OFW 4.80.

Firmware has nothing to do with pulling a dump

Your nor clip cable is either damaged, it's not connected properly or loses proper connection while in use if you're just holding it. Make sure it clips on to the nor chip properly and hold it tight with a g-clamp.
