The patch doesn't count!!
You have to start a game with highest difficulty, then use your difficulty save and beat the last boss.

playing somewhere MP for obtaining the remaining trophy.

Thats works, must have a player data with the hard difficult set and copy the save, after beat the boss the trophy pop up
