Rename it to EBOOT.BIN, and place in your Black Ops directory, overwriting the original. I do not know if this will work on anything besides BLUS30591, but it should.
Steps:
1. Get on a NEW ACCOUNT(CAN NOT HAVE EVER EVEN BOOTED BLACK OPS MP) and then go to Local Splitscreen. Make sure that you can open your friends list by pressing Triangle. If you can, and you see your friends, it will work. Go into the game, and press Select. It should do a fast restart, and you should see some text on the screen. Back out. Open friends list again and look at the right hand corner. If you see 9999999 CoD Points, and a "16th" prestige(it's glitched, it's really 15th) you did it right. You should also be Level 50.
If you don't see "Your stats have been reset" upon clicking Play Online, you did it right. If you do, then your account has played BO before.
Enjoy this while it lasts.
You should also have 17 days played, just to boost legitness(of course, 9999999 Million CoD Points throws this right out the window in terms of legitimacy :p)
By the way, this sticks. 100%. It will never go away unless Treyarch resets you. Restarting BO/PS3 does not affect it.
The real key lies in the old command "statSet". Remember that from CoD4 bypassing? It's technically a variation, as it is StatSetByName, which I don't think was in CoD4.
Also, part of the reason I put this in Non-Premium is because it's basically worthless. Being able to mod stats has absolutely no point.
To be honest I probably never should have released it, but what the hell.
It is supposed to say connecting. You go to splitscreen to do the bind, then you delete gamedata and redownload Black Ops 1.05.

you need to switch the eboots from installed 1.05 patch not the backup manager game directory??
path on mine is //dev_hdd0/game/BLES01031/USRDIR/
not GAMEZ or LAUN12345 etc. sounds like your swapping the main eboot from disc
ps the patch one is about 6mb and the main disc version is 66mb its easy to tell
swap the patch as described above then boot to b-ops normally with disc and follow rest of tut

1. Copied the 6 MB eboot into my black ops directory and replaced it with the 56 MB one
2. Started up black ops through multiman (I was also signed into psn during this)
3. Went to local, splitscreen, checked friends list and it worked, then started a game.
4. Hit autoassign chose the mp5 class and hit select during the countdown and nothing happened.
