Wosley's right, these are PS4 saves, but I can see why you're confused. If you use the Import function, Save Wizard does indeed read Zips and only Zips. It's just that the Import function is not the only way for Save Wizard to resign, plus you can make your own Zips.
If the Import function is easiest for you then make sure the files are in the correct CUSA folder - "CUSA08495" without the quotes - and create a zip from that folder. A ton of free programs can create Zips and I think Windows 10 does it natively (right-click the folder and the drop down menu should have an option for "send to ... Create Zip").
But Save Wizard can also resign without Zips. For this method, first go into your USB stick, into the PS4 folder and savedata folder so that you see the folder containing your profile. It's a string of letters and numbers, I think 16 characters long. If any of your own saves, or saves you've already resigned, are on that USB stick, that's where they would be. Create a DIFFERENT folder to act as a different profile (don't register it with Save Wizard unless you have a separate reason to do so) - like a folder with a bunch of 0s. Again I think 16 characters long but however long your own account folder is. You can then put CUSA folders like this - with the PS4 saves inside of the CUSA folder - directly into that "dummy" folder. Then when you go into the Resign menu you will see Dark Souls Remastered on the menu and you can resign directly from there, rather than the Import button.
