I have tested all saves on a dummy account but if there are any problems, please feel free to let me know and I'll do my best to help out.
Instructions are included in the .zip files.

I have split the file into two separate downloads due to its size.
Also make sure to delete all saves from the PS4 storage before uploading the next ones as there seems to be an issue with overwriting files with the same name. This is caused by the game's save file limit and I can't do anything about it.

Just tried and most of part 2 ( from save 0012 ) Isnt working
Lets just be patient and wait for author to help

I've only tested Part 1 of the save set and can confirm every trophy pops fine, some of the trophies do pop very late so just be patient.
I'm going to start on Part 2 in the next few days when I return back home.

Tho the saves are buggy for me and from save 12 they dont work for me

Hi, guys. Sorry for the lack of response. I've been working nights and haven't had much time.
Now as for the above mentioned issues:
There is a text guide for Part 2 in the folder.
I have tested every save from both parts on a dummy account and managed to pop every trophy. Some do unlock a bit later than others.
Only thing I can think of is that there is an issue when overwriting save files with the same name, that's why only Part 2 is causing problems and 'His Father's Son' save from Part 1 which happens to be number '0014', same as the Honour Mode save.
I suggest deleting all saves from your PS4 storage first and then uploading Part 2. That's how I did it during testing and didn't come across any problems.
Another reason might be the size of the download file. I've uploaded separate .zip files for Part 1 and Part 2. Hopefully that solves it.
I will do my best to check this thread regularly and help with any problems.

But now the last one I need is Infiltration and for some reason I cannot get it to pop after waiting over 30 minutes and unlocking the skull :/
Any idea what should I do with this one?
