If Save Wizard has Advanced Mode for the game - and I don't know if it does - you could try to re-region it. You basically go into Advanced Mode to change a couple of digits around, import and export the values. I'll link to a tutorial below. I see Save Wizard at least has Quick Codes for the game since it's listed on their website but that does not necessarily mean that Adanced Mode is available, and sometimes there's some further bar to re-regioning, such as if the file sizes are too large.
Another option would be searching these forums for the CUSA you need. In the main PS4 Saves forum, you'll see two search boxes in the upper right corner of your screen. The lower of those two searches just the PS4 saves (the upper one searches the whole site). Search for "fallout" or the exact CUSA you are looking for. Doing that just now, I came across one result in your CUSA:
